Elderplan (which includes homefirst - one of the oldest, and today one of the largest and most highly-rated managed long-term care plans in NY state), is a health plan with medicare and medicaid contracts. Elderplan started as one of the original social health maintenance organizations in the country. More than 35 years later, elderplan, a member of MJHS health system, carries on the CORE values of compassion, dignity and respect first established by the four brooklyn ladies in 1907. Elderplan is a not-for-profit health plan committed to supporting, guiding and caring for adults 65 and over and individuals 18+ with special needs in new YORK state, empowering them to achieve their greatest level of independence and quality of life through pioneering health care and social support programs
