Healthy Community Forum for the Greater Sacramento Region operates under the name Sacramento Covered, a not-for-profit philanthropic organization, is an innovative leader in developing and implementing a peer-driven, one-on-one, data-informed approach to connecting families and individuals to the health, behavioral health, social services, and community resources they need to lead healthy, productive lives. Our mission is to improve the overall health and well-being of the Sacramento region by connecting people to health coverage, primary and preventative care, housing, and other health-related supports. Sacramento Covered staff serves the most vulnerable members of our community. SCs model of care aims to promote health equity and improve the well-being of underserved communities by addressing social determinants of health through Community Health Workers.
