The mission of Tapestry 360 Health is to improve the well-being of the communities we serve by providing accessible, high-quality health care. Tapestry 360 Health largely serves the uninsured, underinsured, immigrants and refugees on Chicago's North Side.Sites include seven neighborhood health centers, seven school-based health centers, and three sites where primary care and behavioral health are integrated.
