Medication Therapy Management Empowerment Act of 2015
This bill amends part D (Voluntary Prescription Drug Benefit Program) of title XVIII (Medicare) of the Social Security Act to make a targeted beneficiary under the medication therapy management program of a part D eligible individual who has a single chronic disease of cardiovascular disease, chronic obstructive pulmonary disease, hyperlipidemia, or diabetes.
