Ban Surprise Billing Act This bill places certain limits on the cost-sharing amount for health insurance plan holders for certain services received from out-of-network providers that are based on the median in-network rate for similar services. The bill also establishes requirements related to the practice of billing plan holders for the difference between the out-of-network rate and the in-network rate for certain services; the bill provides for an independent dispute resolution process for services with a median contracted rate of $750 or greater, among other provisions.
