Improving Care for Vulnerable Older Citizens and People with Disabilities through Workforce Advancement Act of 2020 This bill requires the Health Resources and Services Administration to award demonstration project grants to partnerships of health care providers, long-term care facilities, and other organizations to test models of care coordination and service delivery for older individuals, and individuals with disabilities, who have chronic illnesses or are at risk of institutional placement.
