This bill authorizes the Centers for Disease Control and Prevention to establish a Social Determinants of Health Program designed to improve health outcomes and reduce health inequities across America. The program will distribute grants to state, local, tribal, and nonprofit organizations to address social determinants of health—factors like housing stability, food security, and transportation that significantly impact overall health—in target communities with the greatest unmet health and social needs. Eligible recipients will develop evidence-based, community-wide strategies that coordinate among health systems, insurance companies, government agencies, and community organizations to tackle these underlying social issues. The bill also authorizes grants to research institutions and nonprofits to study best practices, provide technical assistance to grant recipients, and share findings across the network. The legislation authorizes $100 million annually for fiscal years 2025 through 2029, with at least 75 percent of funds directed to community grants and research activities, and includes a requirement for an independent national evaluation within five years of the program's launch.
