This bill creates a new Social Determinants of Health Program within the Centers for Disease Control and Prevention to address factors outside of medical care that affect public health, such as housing, food security, and transportation. The CDC Director would coordinate activities across the agency and award grants to state and local health departments, nonprofits, and universities to implement evidence-based strategies that tackle these social needs in communities with the greatest unmet health needs. The program would also fund research to identify best practices and provide technical assistance to grantees. The bill authorizes $100 million annually for fiscal years 2025 through 2029, with at least 75 percent going directly to community grants and research. An independent national evaluation would be required within five years, with results reported to Congress.
