The GAIN Act of 2024 directs the Secretary of Health and Human Services to launch a five-year demonstration project testing whether guaranteed monthly income improves the health of Medicaid recipients. The program would award at least ten grants to local governments, tribal organizations, and nonprofits to provide monthly payments of at least five hundred dollars to eligible Medicaid-enrolled individuals, with no restrictions on how recipients spend the money and no impact on their tax liability or Medicaid eligibility. Participating entities must conduct baseline surveys before the program begins and ongoing surveys at least three times yearly to measure effects on participants' physical and mental health, employment, finances, and spending habits, then report findings to the Secretary annually and to Congress every two years. The bill allows administrative and outreach costs to consume no more than twenty percent of awarded funds, and the Secretary must establish the program within one year of enactment.
