The HEALTH for MOM Act allows states to create "maternity health homes" under Medicaid, which are coordinated care programs for pregnant and postpartum women. Starting two years after the law takes effect, states can voluntarily offer this option, where eligible pregnant women can choose to receive care through a designated healthcare provider, team of professionals, or health team that coordinates all aspects of their pregnancy and postpartum care. Participating providers must be able to coordinate care across multiple specialists and services, develop personalized care plans, and connect women to behavioral health, social support, and community services. The federal government will increase its share of Medicaid payment for these services by 15 percentage points for the first four fiscal years (up to 90 percent maximum), and states must track outcomes including hospitalizations, maternal mortality, and care quality. The legislation authorizes $50 million in planning grants over two years to help states develop and implement their maternity health home programs, and participation is voluntary for both pregnant women and healthcare providers.
