This bill establishes a three-year pilot program requiring the Department of Veterans Affairs to improve care coordination for veterans who are enrolled in both Medicare and the VA health system. The program will operate in three to five Veterans Integrated Service Networks across different geographic areas, including rural and medically underserved regions, and will assign each participating veteran a case manager to develop personalized care plans and help navigate both VA and Medicare services. The VA should contract with private sector health care companies to help design and manage the program, using successful commercial health care models to improve access, quality, and reduce costs while eliminating gaps in care and duplicate services. The Secretary must track detailed metrics on program outcomes including patient satisfaction, quality of care, costs, and care coordination effectiveness, with quarterly reports to Congress during development and results reports throughout and after the pilot program. The legislation aims to serve older veterans and wounded warriors who often struggle to coordinate care between multiple health systems by assigning dedicated case managers to streamline their access to services and medical records sharing.
