This bill would expand Medicare and Medicaid coverage of community health worker services beginning January 1, 2027. Under the Medicare provision, community health agencies would be able to bill Medicare Part B for preventive services and services addressing social determinants of health—such as health coaching, care coordination, transportation assistance, health literacy support, and help accessing resources for food, housing, and employment. Medicare would cover 100 percent of the service costs with no deductible required. For Medicaid, states would have the option to cover similar community health worker services through state plan amendments, and participating states would receive a 6 percentage point increase in federal matching funds to help pay for these services. The bill establishes detailed definitions for community health agencies and workers, emphasizing that agencies should employ workers from the communities they serve, offer market-wage compensation and career advancement, and maintain manageable caseloads to provide person-centered support.
