The bill caps annual out-of-pocket costs for traditional Medicare beneficiaries at $6,700 (adjusted for inflation starting in 2026) and shifts marketplace subsidies to use gold-level plans as the benchmark while extending subsidies to people earning above 400% of poverty level. The legislation strengthens federal oversight of insurance rates to prevent excessive or discriminatory pricing, applies these rules to all group markets and grandfathered plans by 2026, and establishes a $30 billion reinsurance fund for 2026–2028 to help states reduce costs. The bill appropriates $2 billion to establish the new Medicare Part E plans and requires employers without adequate health insurance to direct employees to healthcare navigators.
