The Affordable CHOICE Act would establish a new public health insurance option available to consumers starting January 1, 2027, through existing health insurance marketplaces (Exchanges). This government-administered plan would offer bronze, silver, and gold coverage levels and compete alongside private insurance plans to provide more affordable options and lower costs through competition. The plan would be financed through premiums set by the Secretary of Health and Human Services and would reimburse healthcare providers either through negotiated rates or, if negotiations fail, at Medicare rates. Healthcare providers participating in Medicare or Medicaid would automatically be included in the plan's network unless they opt out, while the plan would require initial start-up funding to be repaid to the Treasury over a 10-year period beginning in 2027. States may establish advisory councils to recommend policies on quality improvement, cost containment, and public awareness of the new option.
