H.R. 6166 expands Medicare's drug price negotiation program by increasing the number of drugs subject to negotiation from 20 to 50 annually and extends the program to private health insurance plans, which can voluntarily participate or opt out while disclosing their status publicly. The bill applies prescription drug inflation rebates to commercial market drugs and requires consideration of international drug prices in negotiations beginning in 2028. Starting in 2027, the bill establishes out-of-pocket spending caps for prescription drugs at $2,000 annually for individual coverage and $4,000 for family coverage across group and individual health plans, with overall limits indexed annually to healthcare premium increases. Additionally, beginning January 1, 2027, health plans must cap insulin cost-sharing at $35 per 30-day supply or 25% of the negotiated price (whichever is less) without deductibles, applying to all major insurance types including employer plans and catastrophic coverage.
