This bill directs the Secretary of Health and Human Services to award three to five grants totaling up to $4.8 million in fiscal year 2026, with funding increasing to $12 million annually by 2028, to establish Community Multi-Share Coverage Programs that provide affordable health insurance and support services to low-income workers at small businesses. The programs target individuals earning above their state's Medicaid limit but no more than 400 percent of the federal poverty line who work for small employers unable to offer affordable coverage, operating through partnerships between nonprofit entities, local hospitals, and small employers. Each program must provide comprehensive health coverage with no deductibles, limited copayments, and integrated services addressing employment barriers, health coaching, and social needs like education and job training. Grant recipients must demonstrate financial sustainability by securing shared contributions from multiple sources including government, healthcare providers, enrollees, and employers, with the goal of reducing dependence on federal funding over time. Programs must be evaluated annually on their impact regarding employment status, health outcomes, income, and economic self-sufficiency, and grants are awarded for four-year periods.
