The Improving Home Dialysis Act of 2026 expands Medicare coverage for patients who perform dialysis treatment at home by adding two new services beginning January 1, 2028: staff-assisted home dialysis respite care and renal mental health services. Staff-assisted respite care allows qualified nurses or technicians to assist patients who are temporarily unable to perform dialysis independently, with coverage limited to 20 sessions per calendar year during the initial 30-day period after starting home dialysis or anytime for patients with physical limitations. Renal mental health services provide mental health support to home dialysis patients during their first 60 days on home dialysis, limited to 4 sessions per patient. Medicare will pay for these new services at rates based on 2025 dialysis training payment rates, with rural areas receiving higher reimbursement rates than urban areas, and these payments will not be made in a budget-neutral manner, meaning they represent new spending rather than offsets to other programs.
